Due to Covid-19, Circare must limit face-to-face contacts to ensure the safety of clients and our staff. For referrals for any of their programs, please call first so inquiries can be directed and addressed appropriately.
Provides case management services to adults with chronic health conditions, serious mental illness, or HIV/AIDS to assist them in staying healthy, out of inpatient care, hospitals, and emergency rooms.

Eligibility: Individuals must: 1) Currently have active Medicaid 2) Have 2 or more chronic health conditions, OR be living with HIV/AIDS, OR have a serious mental illness
Application Process: Referrals accepted from health care providers, community organizations, individuals, and/or family members
Required Documentation: Medicaid ID card and documentation of diagnoses
