Community Networks Day Hab

(Program)

Serves adults with developmental disabilities who are seeking an individualized program within a group setting. Based on input from participants, staff members develop initiatives aimed at personal growth and fulfillment. The focus is on building skills, advocacy, and community involvement. Individuals are encouraged to access community resources to find and strengthen supports and relationships.

Eligibility: Individuals ages 18 years and older with developmental disabilities who are enrolled in the Medicaid Home and Community Based Services Waiver

Application Process: Referrals from individuals, family members, service coordinators, and other professionals
