Due to Covid-19, Circare must limit face-to-face contacts to ensure the safety of clients and our staff. For referrals for any of their programs, please call first so inquiries can be directed and addressed appropriately.
Provides case management services to children with serious mental disturbance or chronic health conditions to assist them in staying healthy, out of the emergency room, and out of the hospital.

Eligibility: Children ages 4 to 21 years with: 1) Active Medicaid 2) A diagnosis of serious emotional disturbance, HIV/AIDS, OR two or more chronic conditions
