Community Habilitation Services

(Program)

Provides community-based, intensive services for adults who are developmentally disabled. Helps them function in the least restrictive environment and increases their personal independence. Encourages individual choice and enhances potential for acceptance into other programs.

Eligibility: Children or adults living in Onondaga County with a developmental disability who are enrolled in the HCBS Waiver
