Non-Medicaid Care Management


Due to Covid-19, Circare must limit face-to-face contacts to ensure the safety of clients and our staff. For referrals for any of their programs, please call first so inquiries can be directed and addressed appropriately.

Provides case management services to children and adults with serious mental illness, to assist them in staying healthy, out of inpatient care, hospitals, and emergency rooms.

Eligibility: Adults and children with a serious mental illness

Application Process: Adult referrals go through Onondaga County Single Point of Access; Child referrals go through Onondaga County ACCESS Team

Required Documentation: Varies; call for required documentation
