Care Coordination of Northern New York


Provides services to both seriously emotionally disturbed children and adults living within the community. Provides holistic management of health care services and reduces avoidable inpatient admissions, emergency room visits, and repeated hospitalizations.

Eligibility: Adults and children with severe mental illness; call for additional eligibility requirements

Application Process: Open referral process; individuals can self-refer or have another person refer on their behalf
