Health Home Care Coordination


Assists individuals with coordination needs such as goal setting, health promotion, transitional care, referrals to community and social supports and patient and family supports, and advocacy.

Eligibility: Individuals must:
1) Receive Medicaid, Medicaid insurance plans, or be eligible to receive Medicaid
2) Have been diagnosed with two or more chronic physical conditions, HIV/AIDS, or serious, persistent mental illness
3) Have significant behavioral, medical, or social risk factors
