Children's Mental Health HCBS Waiver Program


Provides individualized, strength-based approach to serving children with complex, significant mental health needs. Services include individualized care coordination, respite, family support, skill building, intensive in-home services, and crisis response.

Eligibility: Children age 5 to 21 years who reside in Onondaga County and have complex, significant mental health needs

Application Process: Referrals made through the Onondaga County ACCESS Team, 315-463-1100

Required Documentation: Recent psychiatric, psychological, medical, IEP (if applicable), county approval of Waiver LOC
