Maternal and Infant Community Health Collaborative

(Program)

Aims to improve maternal and infant health outcomes including preterm birth, low birth weight, infant mortality, and maternal mortality by building systems of care for women before, during, and after pregnancy. Community Health Workers are available to assist high-need women in accessing healthcare and other essential support services, conduct home visits, and provide group and individual education.

Eligibility: Women of child bearing age and their families in Oswego County

Application Process: Call, walk-in, or go online for services; self- and provider-referral forms available online
