Supported Housing Initiative (SHI)


Provides long-term case management and housing assistance for homeless/unstably housed individuals living with severe and persistent mental illness who need support in order to maintain independent housing in the community. Provides income-based, private apartments with a monthly rental stipend and case management services to assist with personal goals. 

Eligibility: Individuals ages 18 years and older who:
-Have a primary diagnosis of serious mental illness
-Are in the process of relocating from a state-funded program (supervised sites, shelter, hospital, correctional facility)
-Are experiencing substantial impairments in function
-Are homeless
-Have high Medicaid costs or are under an AOT order
-Have the capacity to live independently

Application Process: Referrals are made through a case manager at a shelter or through a counselor at DSS if the individual has a history of mental illness and homelessness; DSS counselors and shelter case managers need to fill out SPOA application in order for referral to go through
