Children's Mental Health Single Point of Access (SPOA)


Process by which children's mental health community-based services are accessed by which level of service recommendations are reached. May be done through a phone screening, child/ family assessment meeting, collaboration with the referral source and other current providers, record review, and presentation to the SPOA Committee for service planning.

Eligibility: Children from 5 to 17 years of age with a psychiatric diagnosis and impairment in functioning due to emotional disturbance

Application Process: Referral packet is submitted to the SPOA Coordinator; records are reviewed to verify eligibility; call for additional information

Required Documentation: SPOA referral packet and parent/guardian signed consent
