Independent Housing Program

(Program)

Provides financial support for security deposits, furnishings, household supplies, and limited additional needs for homeless individuals with serious mental illness. Individuals must meet HUD definition of homeless.

Eligibility: Individuals ages 18 years and older who:
-Have a primary diagnosis of serious mental illness
-Are in the process of relocating from a state-funded program (supervised site, hospital, correctional facility)
-Are experiencing substantial impairments in function
-Are homeless
-Have high Medicaid costs or are under an AOT order
-Have the capacity to live independently

Application Process: Complete a Single Point of Access form (SPOA)
