Receives and investigates complaints of discrimination based on age, race, creed, color, national origin, sex (including pregnancy, breastfeeding, pumping milk, and sexual harrassment), disability, marital status, military status, sexual orientation, genetic testing, arrest record (in employment and credit only), conviction record (in employment and credit only), familial status, and domestic violence victim status (in employment only). Unless otherwise noted, complaints are accepted regarding employment, housing, public accommodations, credit, volunteer firefighters, and use of educational facilities.

Eligibility: New York State residents

Application Process: Persons wishing to file a complaint can download and complete a form available on the Division's website (www.dhr.ny.gov, select "file a complaint") OR contact the office to request a complaint form OR contact the office to arrange a telephone interview appointment

Required Documentation: Photo ID is needed when a complaint is signed and notarized
