Expanded in Home Services for the Elderly Program (EISEP)

Provides assistance to help older adults remain in their homes. Case managers coordinate services, which can include aide service for light housekeeping and personal care, a personal emergency response system, and respite through an adult day program.

Eligibility: Frail elders with chronic physical and/ or mental impairments who are Onondaga County residents and not Medicaid eligible

Application Process: Call for screening

Required Documentation: Call for information

Fees: Cost-sharing for aide service and day programming, depending on income; call for details
