ARC of Onondaga Horizons Clinic
(Program)
Provides social work, psychological counseling, behavior planning services, and physical and occupational therapy within the clinical setting as well as in the home or work environment for individuals ages 4 years and older.
Eligibility: Individual ages 4 years or older with a developmental disability

Required Documentation: Picture ID and proof of insurance
